Please note that the printed acknowledgements to be included in the worship guide on Easter Sunday will be taken directly from these electronic forms, and the paper forms available in weekly bulletins. If you wish to add a note of appreciation or remembrance, please complete the form in its entirety and submit it by Sunday, March 22. Donations or orders received without this form will be added to the total amount without a printed acknowledgement.

Cash, check, and online payments will all be received, however, only acknowledgements submitted via these forms will be included. Please do not include acknowledgements on memo lines or with your online payment.
