Important Forms

As with full-time schools, there are a number of forms necessary so that both you and your child are safe and sound here at St. Paul’s Drop-In Day Care.  The following are forms that need to be filled out and returned so that your child can begin his/her experience here at the Day Care.

The state mandates that Emergency Contact/Parental Consent Forms be reviewed semi-annually (every 6 months). If these forms are not reviewed and signed we are required to remove your child from the program.

Emergency Contact/Parental Consent FormThis is needed in order to register children for the Day Care.  Please make sure ALL areas are filled in.  This form is required by the Department of Public Welfare.

Emergency Plan Letter to Parents This letter contains details of our plan of action in case there is ever an emergency and we need to evacuate the building.  We highly advise all families to read the letter and, if there are questions, address them with staff as soon as possible.

Baby Sheets – Use these forms daily if your child is in the infant area.  The form needs to be filled out with all relevant information so that your child can be taken care of properly by the staff.

Daily sign in sheets – This form needs to be filled out daily and given to the front desk so that the staff has all relevant information so that your child can be taken care of properly by the staff.

Preschool Forms – Please fill this form out if your child is going to and/or coming from preschool.  This form needs to be brought to the preschool and/or child care.

Child Pick-Up Authorization Form – This form needs to be filled out in order to properly register your child for day care.

Consent to use Sunscreen – This form needs to be filled out in order to properly register your child for day care.

Consent for Photography – This form needs to be filled out in order to properly register your child for day care.

St. Paul’s Drop-In Child Care Agreement (2016-2017) – This form needs to be filled out in order to properly register your child for day care.

Getting to Know You – This form needs to be filled out in order to properly register your child for day care.  Even if your child is registered for/has attended the day care for more than one year this form needs to be filled out each year.

Re-Registration Reminders – A list of things that every family should know concerning the drop-in child care and everyday matters.

Child Health Assessment Form – This form must be completed, signed and dated EVERY time your child/children visit the doctor’s for a well-care visit.  The form then must be returned to the day care at the time of your next visit.

Health and Safety Form – This link contains information on St. Paul’s child care health and safety policy.  This also includes the time frames for well-care visits.

Parent Handbook (2016 – 2017) – This handbook contains everything you needed to know about St. Paul’s Child Care and more!

Remind 101 Information – This paper details how to sign up for the Child Care’s Remind 101 updates, receivable through both text messages and email.  Remind 101 is a one-way text messaging and email system that alerts you the parents/families to anything crucial happening at Child Care, such as if we have to close due to the affiliated school districts closing.  PLEASE do sign up for this service; it’s free!
